Appointment Request
If you're part of a health plan or have special health care coverage, please have this information available when you call for your appointment.
It's your responsibility to bring your insurance card, co-pay and, if required, your referral papers from your primary care provider when you come for your appointment.
In the event you are unable to keep a scheduled appointment, please advise our receptionist as soon as possible. This allows us to keep a flexible schedule for all of our patients.
